Difficulty 0 Difficulty 1 - Awakened Difficulty 1 - Adaptive Difficulty 3 - Fused Difficulty 4 - Refined
Players often refer to these as D0 through D4 when referring to difficulty levels.

D0 or 'difficulty zero' is the easiest way to play the game. Any combination of classes can be effective in this difficulty.
Base level (+0) loot drops in D0, with a chance to get higher level items. If you change the difficulty to higher than D0, you will get higher level wearable items (armor, weapons, jewelry, etc.) and motes to drop in that zone.
For example, if you change the difficulty to D4, you will get +4 wearable gear and +4 motes to drop every time, with a chance to get even higher-level versions to drop.
Changing the difficulty level to be higher also means that you will face harder enemies with a larger aggro range and the enemies have a higher chance of being 'multi-class' just like players. The named mobs in D2 are often multi-class. Raid bosses will start appearing as triple-class on D3. And on D4, the aggro range is nuts but sometimes this this works to your advantage if you are looking for a challenge, especially if you are in a group.
